WHAT YOU NEED FOR LIVE SCAN?
Applicants must bring a completed “Request for Live Scan Service”, a 3-part applicant submission form. This form must be filled out with information provided in all required fields, including Social Security Number. We can’t help you fill it out, and it is required by the agency that referred you for live scan fingerprints.
If you have not been provided with this form, request one from your referring employer, licensing volunteer organization, or referring agency.
For our clients’ convenience, we have blank “Request Live Scan Service Forms” at our site. However, it is your responsibility to obtain the necessary ORI Code, Agency Set Address and Mail Code that pertains to your referring agency.
Bring a valid photo ID which may be one of the following: CA Driver’s License, CA DMV ID card, Out-of-State Driver’s License, Passport, Alien Registration Card (green card), Military ID card (not expired) or Mexican Consulate ID card.
PAYMENT INFORMATION
Payment for Live Scan service can be in cash, check, money order, or cashier’s check.. Billing accounts are accepted.
EXCLUSIVE USER AGREEMENT
We will reduce our rolling fee based upon an established number of fingerprint scans. If you agree to send all of your applicants to our company exclusively, we will reduce our rolling fee for you.
